Athlete Code of Conduct

Introduction
BullTrained MMA LLC is committed to excellence on and off the wrestling mat. BT expects that through participating in our beloved sport and engaging in team development activities, its athletes will experience personal growth and athletic success.

Being a member of BT is a privilege, not a right. Each athlete is required to acknowledge and accept this Athlete Code of Conduct as a condition of participating at the BullTrained High-Performance Center.

Code of Conduct
By signing below, I agree to conduct myself in accordance with all rules and requirements of BullTrained MMA LLC.

I also agree that:

1. BullTrained Community
I will strive to represent BT, my team, and myself well, on and off the mat, on and off campus; to be a responsible, engaged member of the BullTrained community; to exercise good judgment; to conduct myself with honesty, integrity, and respect for others; and to lead by example.
- I am responsible for keeping my area clean from food debris and trash, making my bed daily, taking my dirty clothes to the laundry, putting away my clean clothes, helping to clean mats after workouts, taking care of the gym equipment, hanging my towel to dry, and cleaning any mess that you make.
-At BullTrained if you break it, you by it. This is our home, please don't break TVs, doors, or windows, punch holes in the walls, throw away dishes, or destroy furniture. You can not add bubbles, salt, any anything at all to the water in the hot tub... again, if you break it, you buy it. Please be aware, we have cameras around the premises and inside the facility to protect you and us.

2. Ethical Conduct
I acknowledge my obligation to, “promote the character development of participants, to enhance the integrity of higher education and to promote civility in society, athletes, coaches, and all others associated with the BullTrained Program and should adhere to such fundamental values as respect, fairness, civility, honesty, and responsibility.”

3. Online
I will remember that, as an athlete, my online activity will reflect (either positively or adversely) on me, as well as on my team and the BullTrained brand. I understand that my online actions are subject to the same rules and standards that govern all other actions.

4. Alcohol and Drug Use
Regardless of my age, I will not consume alcoholic beverages or controlled substances while involved in any team-related competitions, camps, travel, or other activities. At other times, I will comply with applicable laws on the use of alcohol and controlled substances. I will not use or distribute illegal, prescription, or “performance-enhancing drugs,” or take prescription drugs unless they have been prescribed for me by a medical professional.

5. Violence
I will not engage in and will not tolerate violent acts, including assaults on persons or property, hate crimes, hazing (addressed more fully below), stalking, sexual violence, or any other conduct prohibited by law or USA Wrestling policy. If I witness any such acts perpetrated by others, I will report them to my coach or another camp counselor official immediately. I acknowledge that BullTrained may, in its discretion, immediately suspend me from my team and prohibit me from practice and competition until further notice if I am accused of a crime of violence, including sexual assault.

6. Nonconsensual Sexual Contact
I will not initiate or engage in nonconsensual sexual contact or sexual harassment, and I acknowledge that all forms of harassment and non-consensual sexual contact are prohibited both by law and USA Wrestling policy.

7. Hazing
Hazing is prohibited by law and the Code of Athlete Responsibilities. I will not participate in hazing of any kind. I acknowledge that such acts, either in connection with a BullTrained camp or competition, participation in an informal or formal team activity or for any other reason, are strictly prohibited. I will refrain from any act, whether physical, mental, emotional, or psychological, that subjects another person, voluntarily or involuntarily, to anything that may abuse, mistreat, degrade, humiliate, harass, or intimidate another person. Such acts may include but are not limited to forcing, requiring, or pressuring others to consume alcohol or any other substance; forcing, requiring, or pressuring others to involuntarily perform physical activities, tattoo, pierce, or shave any part of the body; forcing, requiring, or pressuring others to take part in illegal or indecent activity; disturbing others during normal sleeping hours; or physically abusing others in any way.

8. Remaining Informed of Expectations and Policies
I am responsible for my own behavior and for reading, understanding, and abiding by the policies applicable to me, including but not limited the Code of Athlete Responsibilities.

I understand that failure to comply with the conduct expectations described in this BullTrained Athlete Code of Conduct may result in sanctions, up to and including suspension or dismissal from the team or camp, and suspension or expulsion from BullTrained High-Performance Center without a refund. If you are suspended, you will still have to pay any and all remaining balances owed to BullTrained MMA LLC.

By signature, with full knowledge of the facts and circumstances surrounding the ACTIVITY, I acknowledge my child’s participation in the
ACTIVITY may expose him/her to actions, events, and environments that may be hazardous to his/her person and/or property. I
acknowledge that I am solely responsible for any action that my child may participate in associated with this ACTIVITY or around this
ACTIVITY, regardless if occurring before, during or after the period of the ACTIVITY.
I have adequate applicable insurance necessary to provide for and pay any medical costs that may directly or indirectly result from my
child’s participation in the ACTIVITY, or otherwise understand that I am solely responsible for any medical costs that may directly or
indirectly result from my child’s participation in the ACTIVITY. I understand that I must have an active, USA Wrestling participant insurance.
I will indemnify and hold BullTrained MMA LLC., acting by and through the BullTrained High Performance Summer Training Camps, it employees, directors, officers, and agents (hereafter referred to as BullTrained) harmless with
respect to any and all claims, injuries, and costs associated with my child’s participation in this ACTIVITY.
It is my express intent that this Acknowledgement of Risk and Waiver of Liability shall bind my spouse, the members of my family and my
estate, heirs, administrators, personal representatives and assigns. I further agree to save and hold harmless, indemnify and defend BullTrained from any claim by the aforementioned parties arising out of my participation in the ACTIVITY.
I recognize and acknowledge that BullTrained makes no guarantees, warranties, representations, or other promises relative to the
ACTIVITY, and assumes no liability or responsibility for injury or property damage that my child may sustain as a result of participation in
the ACTIVITY.
I further understand and agree that this is a release of liability and indemnity agreement, and it is intended to be as broad and inclusive as
permitted by law. If any portion hereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full force and legal
effect.
SIGNATURES
In signing this Acknowledgement of Risk and Waiver of Liability I hereby acknowledge and represent: (a) that I have read this document in
its entirety, understand it, and sign it voluntarily; and (b) that this Acknowledgement of Risk and Waiver of Liability is the entire agreement
between the parties hereto and its terms are contractual and not a mere recital. Further, I certify that I am the parent or legal guardian of
the above-named participant in the ACTIVITY. On behalf of myself and my spouse, partner, co-guardian or any other person who claims
the participant as a child. I acknowledge that my child and I have agreed to the terms and conditions of my child’s participation in the
ACTIVITY, and I hereby give my consent to participation by my child in the ACTIVITY, and to receive medical treatment determined to be
necessary. I further agree to hold harmless, indemnify and defend BullTrained from and against all claims, demands or suits that my
child has or may have.

I hereby consent to be interviewed, recorded, photographed, videotaped or filmed by representatives of BullTrained MMA LLC. for purposes of publication, display or broadcast (print, web, digital display and all other forms of media).

I agree that such interviews, recordings, articles, quotes, photographs, films, audio or video and/or any reproductions of same in any form, are the property of the BullTrained MMA LLC., and I relinquish any present or future claim for reimbursement for said photographic or film reproduction of my likeness or for said testimonials by me.

I hereby release BullTrained MMA LLC., its affiliates, employees, representatives and agents from any and all claims, demands, costs and liability that may arise from the use of these interviews, recordings, photographs, videotapes or films, and/or any reproductions of same in any form, as described above, arising out of being interviewed, recorded, photographed, videotaped or filmed.

I acknowledge that I have read this consent form in its entirety, or it has been read (or translated) to me, and I have had the opportunity to ask questions about it and understand it.

I hereby certify that I am legally indebted to BullTrained MMA LLC and I hereby promise and bind myself to pay the full amount agreed upon during camp registration. The amount I owe has been laid out via the first invoice sent to me by BullTrained MMA LLC and by paying the first invoice I legally bind myself to the payment plan contract.
If the full amount owed is not paid by the contract deadline, BullTrained reserves the right to add either a monthly interest fee of 10% or a one-time fee of 25% of the remaining balance.
BullTrained is dedicated to family and will do all it can to work with their families, but does reserve the right to take legal action if needed to collect debts.
